The Acamarian ship owned by the rogue "Gatherer" faction of Chorgan from TNG: "The Vengeance Factor" is the first re-use of the Pakled ship from TNG: "Samaritan Snare". There are no obvious modifications to the miniature. Only the color was changed to bluish gray, probably just through the lighting or post-processing. The two might be the same design.

The pod was used in "Proving Ground" and could be briefly seen at the end of the episode when Archer returned to Enterprise. It is possible that the model re-uses parts of the CGI of the big Andorian warship.

These Andorian starships could be seen in ENT: "Cease Fire", "Proving Ground", "Zero Hour", "Kir'Shara" and in the Andorian trilogy beginning with "Babel One". In "Kir'Shara" they appeared to be considerably smaller than the Vulcan ships of the D'Kyr type, which are consistent at some 600m. A size chart by CGI wizard Rob Bonchune reveals that the ships is supposed to be 360m long. This is reasonable and would give us a deck height of about 3.5m.
Shran stated in "Babel One" that his ship was the first of her class. This is why we can refer to it as Kumari class. The name honors the first ice cutter to circumnavigate Andoria. The crew complement of (only) 86 is from this episode too.
The type, size or capabilities of these vessels remain unknown. We only know that they are equipped with beam weapons.

The second type of Andorian vessels could be seen since ENT: "Kir'Shara", where they approached the site of the battle alongside the already familiar Kumari-class warships (above). None of these ships was shown up close. The design is essentially a re-use of the Valakian sublight ship from ENT: "Dear Doctor", but now with a hull extension in the center, with elongated wings, pods like on the Kumari on the wingtips, blue engines and a bluish gray hull color. It looks like the ship type 2 is smaller than the Kumari.
The Angosian transport vessel carries this type of small escape pods. At least one of them is stored in the ship's engineering section.

A tube with a moving light inside, obviously representing a train, can be seen in the cityscape on Angosia III.
The Arkonian shuttle appeared in ENT: "Dawn". The vessel may be warp-capable, but more likely it was released from the Arkonian warship to intercept Trip's shuttle. It was also him who called the shuttle a "patrol ship", which was obviously an overstatement.
The CG model previously appeared as "Alice" in VOY: "Alice". It was modified for the Arkonian shuttle, such as with smaller side windows. We couldn't see the full-scale mock-up very clearly in the ENT episode, but there are slight discrepancies when compared to the CG version. We can assume the shuttle is still the same size as "Alice".
This Axanar vessel fell victim to the unidentified "bodysuckers" in ENT: "Fight or Flight". The ship was designed by John Eaves.
John Eaves prepared a size chart for the ships in the the episode. Regarding the Axanar cargo vessel, my own estimation of 181m, compared to the docked Enterprise shuttle, is close enough to the originally intended length of 198m. The Official Starships Collection for some reason estimates this ship to be only 92m long, which is far too small.
The second Axanar vessel appeared towards the end of ENT: "Fight or Flight". While the weapons of Enterprise (prior to the installation of phase cannons) were no help against the huge ship of the "bodysuckers", the beam weapons of the Axanar were capable of penetrating their shields. The second Axanar ship shares a lineage with the first one, but the essential parts, like the command section, the central section and the engines are different.
On the original size chart by John Eaves this ship is 1.37 times as long as the Axanar cargo vessel. I stick to the size ratio in a lack of size references in the episode itself.
No further details are known about this type of freighter which is equipped with warp and impulse propulsion and with low-level particle phasers. Article: Redresses of the Husnock Ship

The Kallisko's crew was killed by the crystalline entity in TNG: "Silicon Avatar". All shots of the ship are stock footage of the Kivas Fajo's Jovis from TNG: "The Most Toys", so it is obviously the same type as a Zibalian freighter.
Jaheel was the stubborn captain of this ship who ignored the quarantine in DS9: "Babel" and tried to escape with his ship, thereby overloading the engines. His race or affiliation was not mentioned in the episode, but a comparison with the Boslic captain in "Sons of Mogh" demonstrates that Jaheel was a Boslic too.
The model looks like a ship that had been built by Kenneth Larson for "Buck Rogers in the 25th Century" (external link), with some modifications to make it look bigger. Curiously the miniature of Jaheel's ship was the basis for the CGI of Neelix's shuttle.
The Boslic freighter was docked to Deep Space 9 in DS9: "Sons of Mogh". Since the Talarian patrol ship and warship look like closely related designs, the various later incarnations of the characteristic warships may be of Talarian origin. They are mostly used as freighters or science ships, though. All takes of the Boslic ship are stock footage of the Valerian freighter Sherval Das from DS9: "Dramatis Personae", so at least these two designs must be identical.
A Corvallen freighter was destroyed by a Romulan Warbird in TNG: "Face of the Enemy". The whole appearance of the ship is stock footage of the Straleb vessel from TNG: "The Outrageous Okona", partially mounted into a Romulan viewscreen. Therefore, as opposed to the incorrect drawings in the Fact Files, there is no difference between the two ships. Between the two episodes the studio model had been modified to become the Angosian ship (see above).
This is an autonomous probe, equipped to modify either technology or brain functions in a way to show alien civilizations the way to the Cytherian homeworld. Irregularly shaped channels on the surface light up when the probe is activated. The probe is impervious to phaser fire. Its propulsion system is unknown, it exhibits neither particle emissions nor subspace distortions.

The probe appeared in TNG: "The Nth Degree". The Cytherian homeworld is presumably located in the center of the galaxy (which could be in any of the four quadrants, strictly speaking).
A brownish shuttle transferred Feezal, one of Phlox's wives, to Enterprise. It was most likely a shuttle of the planet Dekendi III where the conference was taking place. Another, rather gray shuttle of the same type could be seen on the ground. It was a bad choice to re-use the early Ferengi ship, which had been said to be of an unknown design. Clearly the two may not be related.

The rails leading to the conference hall point to the existence of a train system.
The shuttle appeared in a parallel reality in ENT: "Twilight", and it was implied that it carried Phlox to Ceti Alpha V. The origin of the design is uncertain, but since he came directly from Denobula, it is most likely Denobulan. It may well exist in a better future too. The model appears to be a re-use of the Mazarite shuttle.
Another shuttle that previously transferred Phlox's wife Feezal from Dekendi III (see above) to Enterprise was a re-use of the early Ferengi ship, the Ferengi being an unknown race at that time. It would only unnecessarily complicate the matter if we claimed this was another Denobulan vessel.
A Denobulan medical ship appears in ENT: "Cold Station 12" and is intercepted and hijacked by Soong's Augments to gain access to Cold Station 12. After they have escaped at the end of the episode, they tow the ship aboard the shuttlebay of their Klingon Bird-of-Prey. Eventually, in "The Augments", they return the Denobulan pilot to her ship, release it from the shuttlebay and drop it into the atmosphere of a gas giant.
The 22nd century Klingon BoP is itself a tiny ship, not larger than its 23rd counterpart. It measures 115m according to my estimations. The shuttlebay as seen in the two episodes (the newer BoP has none that we know of) is 13.6m wide and 4.7m high at this scale. It would allow to hold three standard shuttles, side by side. But can the Denobulan ship possibly be that small? The design doesn't look like that at all. The spacious bridge set with two seats, with a comfortable ceiling height and with what looks like access to an aft section would not fit in, considering that the usable space of an only 4.7m high vessel, without parts that clearly belong to the engines, is only some 1.8m wide and 2m tall at its highest point. This is a blunder not only in the VFX but already in the plot. There are likely more crew members supposed to be on the ship than just the pilot anyway.
The Denobulan medical ship looks a bit like the Triannon vessel (both are vertically oriented and share some of their lines) but is a different design.

The Ekosian missile appeared in TOS: "Patterns of Force". The footage is of a German V-2 rocket (quite purposely, seeing how the planet is modeled after Nazi Germany). Only that the V-2 was neither carrying an atomic warhead nor did it have interplanetary capability.
In the remastered episode we can see a CGI rocket from a distance on the Enterprise's viewscreen instead of just a red light. It is unknown whether the CGI model is an authentic reproduction of a V-2.

Among other changes and refinements, two previously non-existent tracks of what appears to be a system of hovertrains were inserted into the matte painting of Eminiar VII for TOS-R: "A Taste of Armageddon".
This is a small ship carrying prisoners to Enolian detention colonies. There is apparently only one inhabited deck on which the prisoners are sitting in rows like on an ancient galley. The ship has at least three aft cannons, but is weakly armed compared to patrol ships.

The ship appeared in TOS: "Spock's Brain". It was a poor choice to call the propulsion system an "ion drive", since such a technology (which will be available in the real world soon) would not provide FTL capability. There must be something else about it to make a so equipped ship even faster than with conventional warp drive. On the other hand, it was correct in the episode that such an engine would leave a trail of charged particles that could be tracked.
The ship must be more than 10,000 years old, considering that the Eymorg wouldn't have the technical knowledge to build it. It must also be a miracle of easy maintenance, considering the Eymorg have absolutely no idea how their technology works.
I wonder if the modeler who built this vessel was aware of what it looks like (it was in the 1960s after all). Or could it have been an intentional in-joke, the Eymorg being a female-only civilization? ;-)
The Eymorg ship was completely redesigned for TOS-R to a ball with attached engine pods. The rationale, according to CBS: "...The comparison between the original and the remastered version provides two totally different looks, with styles defined more by the contemporary technology. The original is a standard rocket-shaped configuration. At the time, 'spaceships' tended to fall into a one of two classic looks, the other being the flying saucer. The new, remastered version of the ion propulsion craft is smaller and more utilitarian, reflecting its single-person occupancy, and it ditches the unnecessary missile/rocket configuration. The design also reflects more modern-day ion propulsion prototypes." Aside from avoiding the dildo comparison, this official justification neglects that, with its rocket pods modeled after 21st century ion drive concepts, the remastered ship actually looks rather outdated than the original, and not at all like the highly advanced vessel that the script called for. Also, it is notable that "rockets" are a rarity in TOS and have appeared in larger numbers only lately in Star Trek Enterprise.
